Please be advised that appointments will only be made AFTER receiving completed new patient paperwork. Our office must process your paperwork prior to seeing you the first time in order to ensure that the doctor and the staff have all of the needed information to provide the best possible care for you. This process will also help you spend less time in the waiting area, streamlining your initial visit.
You may obtain new patient paperwork by coming by our office and picking it up. You may either fill out the paperwork in the waiting area in our office or bring it back once it is completely filled out. Again, everything on the new patient paperwork has to be filled out completely before making any appointment.
In order to be more convenient to our new patients, we also located new patient paperwork packet at the bottom of the stairwell on both north & south entrances to our office, so that you can come and pick it up any time of the day and week.
